I've worked in a variety of IT shops and have had the privilege to work with a number of other people across industries. Blame culture is often at the heart why environments are dysfunctional and fragile. Because of the threat of consequences from mistakes, where even being in the vicinity of an outage can impact one's career prospects, plausible deniability has been raised to an art form - covert operators and their task masters could learn a lot from IT Ops personnel in enterprise. Obfuscation of action and distancing oneself from incidents are core enterprise IT skills.Beyond Blame highlights this, in the same way that the Phoenix Project introduces DevOps and how Lencioni tackles team dysfunction. Much like the series of books from Lencioni, Beyond Blame starts with a fictional account that most of us who worked in IT can relate to. The book then progresses to provide actionable steps for transitioning to learn from failure without the caustic consequence of blame and reprisal.Where I currently work, we conduct postmortems for almost every significant event. For a time, I struggled to understand the importance or value to these - as my previous experience with postmortems were targeted to find the responsible person, not for learning how to make things better. Zwieback's treatment of the purpose of postmortems, as a learning tool that can help us make more resilient systems, cemented for me why we do them and why they are important.

This book is a story about moving from a culture of blame - and the costs of a culture of blame - to a culture of blamelessness. What does that look like?Problems with a culture of blame: accountability means finding a scapegoat, which means that people fear sharing what they know because the repercussions are so extreme. And in any complex system, it's hard to understand what is going on and how things will interact. Stuff will happen - when people don't share what they know, it can't be learned from. When people are fired because they did their best but were operating with incomplete knowledge and something broke as a result, organizational knowledge is lost.The main process outlined in the book for a blameless culture is the blameless postmortem. People share what happened and what they know, and are not judged (or allowed to judge themselves) with the benefit of hindsight. Then things that need to be followed up on can be identified.Really the main message I took from the book is that blamelessness is necessary for a culture of accountability - because without blamelessness, it's not safe for people to share the full details of what happened, which means no-one really knows what did happen.

The business novel approach evokes for me classics such as The Goal and The Phoenix Project. It's a good way to frame and illustrate the process of organizational transformation. In this one, a team of mid-level managers have came to understand that postmortems should be focused solely on learning and a way to develop a better understanding of the complexity of the systems.I enjoyed this book; it was a breeze to read through, but is clearly well researched and covers the material well. I especially enjoyed how the author integrated multiple concepts such as Cynefin, cognitive biases, and counterfactuals into the book, and gave an example of running through a blameless postmortem. The last chapter is a summary / guideline how to run a blameless postmortem.The one thing I wish the author covered, though arguably beyond the scope of the book, is how to prioritize remediation. The book makes the point that not all remedies would be cost-effective to implement, which makes sense, but would have been great to see an illustration of that.It would have also been good to see a conclusion - a short chapter about how the organization changed after six months.

Incredible integration of transformational knowledge for any organization in an engaging story! So many of the ways we need to grow together are supported in this easy to read book. Thank you for this brilliant approach to creating a truly effective learning culture. I can't wait to share this with so many stuck in ineffective and punitive cultures traditionally blaming their people as problems to control. People as solutions to harness in exploring our volatile, complex, uncertain, and ambiguous environments are our best hope! This book shines light on the new way forward. Nicely done sir! Thank you!

Excellent short narrative on how understanding complex systems as they relate to IT environments. Authors do a nice job of setting up a relatable scenario and introduce the concepts with familiar context. Highly recommend reading this for anyone involved in IT operations or application development, especially in organizations that are employing DevOps principles.I do recommend having the authors add a FAQ appendix to the book based on feedback that they receive from their readers. I'm sure they've addressed many common questions from IT professionals. They could even extend the town hall section of the story to include more of these.

Easy read--finished in less than a week, but the content was spot on. This book addresses an all too common issue where people are penalized for telling the truth about problems. I had our team read this as a group and the transition and openness about problem areas was greatly improved

Dave’s approach to handling these failures shows his depth of knowledge, understanding, and leadership in dealing with complex IT issues. Often times, its simpler to cast blame without truly understanding “root cause” only to have the same failures reoccurs at later times. The author points out that understanding “root cause” means cultivating an environment and culture where IT folks are not afraid of being reprimanded or penalized for making a mistake or being accountable. Really great read!
